House Bill 1586 amends the Health and Safety Code to revise provisions relating to the affidavit form developed by the Department of State Health Services (DSHS) for use by a person claiming an exemption from a required immunization based on reasons of conscience by requiring DSHS to post the form online in a printable format, removing a requirement for the form to contain a seal or other security device, and establishing that a person may not be required to provide any information to obtain a blank affidavit form. The bill also revises provisions relating to the maintenance of records by DSHS with respect to the affidavit form, including by removing the requirement for DSHS to maintain and report to the legislature a record of the total number of affidavit forms sent out each year.
