            Senate Bill 1788 amends the Education Code to require the Texas Education Agency, not later than December 1, 2011, to develop a model form for use in developing an individualized special education program for a student with a disability. The bill sets out requirements for the information contained in the form, requires the agency to post the form on the agency's Internet website, and authorizes a school district to use the form to comply with the requirements for an individualized education program under federal law. The bill limits the information that may be required in the written statement of a student's individualized education program to the information included in the model form.

            Senate Bill 1788 requires the appropriate state transition planning for a student who is enrolled in a special education program to begin not later than when the student reaches 14 years of age.
