Amend CSHB 3 on page 12, between lines 2 and 3, by inserting
the following:
      (d)  The participation criteria for authorized insurers,
health service organizations, health maintenance organizations, and
other entities that provide coverage under the health benefit
program may not require that an eligible coverage provider provide
benefits for services rendered in a particular location or in a
particular manner.
