Amend HB 2061 as follows:
      (1)  In SECTION 1 of the bill, page 3, line 1, strike "form
of a".
      (2)  In SECTION 1 of the bill, page 3, lines 2-3, strike
"distribute copies of the form and applicable rules to" and
substitute "notify".
      (3)  In SECTION 1 of the bill, page 3, line 3, between
"higher education" and ".", insert "of the required statement and
the applicable rules".
      (4)  In SECTION 1 of the bill, page 3, line 3, strike "form"
and substitute "statement".
