Amend HB 3016 as follows:
      (1)  On page 4, line 11, strike "<and>" and substitute "and".
      (2)  On page 4, line 19, between "physician" and "making",
insert "or other health care provider".
      (3)  On page 4, line 24, strike "review; and" and substitute
"review".
      (4)  On page 4, lines 25-27, strike Subdivision (7), as added
by the bill.
