1-1 SENATE RESOLUTION NO. 361 1-2 WHEREAS, The Senate of the State of Texas takes pleasure 1-3 in recognizing Sally Collins of the Texas Department of Human 1-4 Services, who was recently recognized and approved for an award 1-5 by the Texas Incentive and Productivity Commission for her winning 1-6 suggestion; and 1-7 WHEREAS, The proposal made by Sally Collins was to revise 1-8 the Medicaid Action Notice (Form 1122) to include a section for 1-9 comments, thus simplifying the use of some forms and possibly 1-10 saving the state money; and 1-11 WHEREAS, The award for submitting a winning suggestion 1-12 is a certificate of appreciation from the commission and, if the 1-13 suggestion saves money, a cash award; and 1-14 WHEREAS, In submitting her suggestion, Sally Collins has 1-15 made a real difference, while demonstrating her ingenuity and 1-16 initiative; and 1-17 WHEREAS, Dedicated and talented, Sally Collins is to be 1-18 complimented for her conscientiousness; now, therefore, be it 1-19 RESOLVED, That the Senate of the State of Texas, 1-20 76th Legislature, hereby congratulate Sally Collins for winning 1-21 her State Employee Incentive Program award; and, be it further 1-22 RESOLVED, That a copy of this Resolution be prepared for 1-23 her as an expression of the high regard and esteem of the 1-24 Texas Senate. 2-1 Fraser 2-2 ______________________________________ 2-3 President of the Senate 2-4 I hereby certify that the above 2-5 Resolution was adopted by the Senate 2-6 on March 8, 1999. 2-7 ______________________________________ 2-8 Secretary of the Senate 2-9 ______________________________________ 2-10 Member, Texas Senate
