Amend HB 1115, page 5, line 7, new section 707.006,
subsection (c), by adding a new number (10) and renumbering
subsequent numbers accordingly:
      (10)  a preprinted form, with self-addressed, postage-paid
return envelope, that can be signed and returned by the owner
stating the owner's request for an administrative adjudication
hearing.
