Amend the Eiland amendment to CSHB 1862 as follows:
      (1) On page 18, line 27, between "after" and "the" insert
"receipt of a claim or 45 days after receipt of a completed
attachment from the physician or provider, whichever is later".
      (2) On page 18, lines 27-28, strike "date the claim is
received by the health maintenance organization".
      (3) On page 19, line 15, between "the" and "attachment"
insert "completed".
