Amend the proposed floor substitute to CSHB 1862 as follows:
      (1)  On page 16, line 21, between "confirmation." and "If"
insert "The physician or provider's clearinghouse must be able to
verify that the filing contained the correct address of the entity
to receive the filing.".
      (2)  On page 16, line 24, at the end of (d) add "The
commissioner shall promulgate a form to be submitted by the
physician or provider which easily identifies all claims included
in each filing which can be utilized by the physician or provider
as their log.".
