	(1) On page 5, line 23, strike "and".                                   
	(2) On page 5, line 27, between "application" and the period, 
insert: 
	;                                                                       
		(7) the type of contamination on the property for which 
the designation is sought; and
		(8) identification of the party responsible for the 
contamination of the property, if known
