Amend SB 310 as follows:                                                     
	(1) On page 3, strike lines 7-8 and substitute "(c) The 
insurer shall file, in a format specified by the commissioner, 
including an electronic format:"
	(2) On page 3, lines 23-24, strike Sec. 3(d) in its entirety 
and renumber the subsequent sections appropriately.
	(3) On page 3, lines 25-27, strike Sec. 3(e) in its entirety 
and renumber the subsequent sections appropriately.
	(4) On page 5, line 18, after the period add "The report may 
be created based on a sample of the information provided under Sec. 
3 of this article."
