Amend CSHB 384 as follows:                                                   

(1)  On page 1, line 21, immediately following "that", insert 	":
			(A)".                                                                 
	(2)  On page 1, line 23, strike "court" and substitute 
"department".
	(3)  On page 1, line 24, between "individual" and "with", 
insert "who has a significant, long-standing relationship with the 
child or the child's family".
	(4)  On page 2, line 1, between "placed" and the period, 
insert the following:
	; and                                                                   
			(B)  informs the parent, conservator, or other 
custodian of a location that is available to the person to submit 
the information in the form 24 hours a day either in person or by 
facsimile machine or by e-mail
