Amend SB 1149, as amended on second reading, in SECTION 1 of 
the bill as follows:
	(1)  On page 3, line 14, after "in the ordinary course of 
business" add "and sufficient for the provider to determine at the 
time of the enrollee's visit information".
	(2)  On page 3, line 11, between "shall" and "make" insert ", 
upon the participating provider's submission of the patient's name, 
relationship to the primary enrollee, and birth date,".
