A BILL TO BE ENTITLED
AN ACT
relating to certain information required to be contained in an application for or renewal of a hospital license. BE IT ENACTED BY THE LEGISLATURE OF THE STATE OF TEXAS: SECTION 1. Section 241.022, Health and Safety Code, is amended by amending Subsection (b) and adding Subsection (f) to read as follows: (b) The application must contain: (1) the name and social security number of the sole proprietor, if the applicant is a sole proprietor; (2) the name and social security number of each general partner who is an individual, if the applicant is a partnership; (3) the name and social security number of any individual who has an ownership interest of more than 25 percent in the corporation, if the applicant is a corporation; and (4) any other information that the department may reasonably require. (f) The department shall post on the department's Internet website a list of all of the individuals named in applications as required by Subsections (b)(1)-(3). The department may not post on its Internet website a social security number of an individual required to be named in an application under Subsections (b)(1)-(3). SECTION 2. Section 241.023(b), Health and Safety Code, is amended to read as follows: (b) A license may be renewed annually after payment of the required fee and submission of an application for license renewal that contains the information required by Section 241.022(b). SECTION 3. The change in law made by this Act applies only to an application for an original license or a renewal of a license submitted on or after the effective date of this Act. An application for an original license or a renewal of a license submitted before the effective date of this Act is covered by the law in effect on the date the application was submitted, and the former law is continued in effect for that purpose. SECTION 4. This Act takes effect September 1, 2005.
