Amend CSHB 1060 as follows:                                                  
	(1) On page 1, line 7, strike "Subsections (i) and (j)" and 
substitute "Subsection (i)".
	(2) On page 1, line 10, strike "(i), and (j)" and substitute 
"and (i)".
	(3) Strike page 1, line 17 through page 2, line 16, and 
substitute the following:
	(i) The prohibition provided by Subsection (b)(1) does not 
apply to a hospital that accesses or uses the information to 
complete a form that is required to provide health care services to 
the individual who holds the driver's license, commercial driver's 
license, or personal identification certificate.
