	Amend CSSB 82 in SECTION 1 of the bill, in added Section 
33.203(c), Education Code (page 3, line 21 through page 4, line 6), 
by striking Subsection (c) and substituting the following:
	(c) The Texas Medical Board shall adopt a form to be used as 
the "Preparticipation Physical Evaluation--Medical History" form.
