	Amend SB 1879 on third reading, in SECTION 2 of the bill, by 
striking amended Section 481.074(k)(3), Health and Safety Code, and 
substituting the following:
		(3)  the name and address of the patient or, if the 
controlled substance is prescribed for an animal, the species of 
the animal and the name and address of its owner;
