Amend HB 1634 as follows:                                                    
	(1)  On page 4, line 24 between "by" and "electronic", insert 
"or "by other means".
	(2)  On page 5, line 6, strike "and certifies that" and 
substitute "or a letter of authorization from the insurer 
authorizing obtaining the report for insurance purposes only that 
certifies".
