Amend CSHB 1 (house committee report) in Article XI of the bill by adding the following appropriately numbered rider after the appropriations to the Department of State Health Services (page XI-5):

____. Appropriation for Diabetes Prevention and Control. In addition to other amounts appropriated to the Department of State Health Services in Strategy A.3.1, Chronic Disease Prevention (page II-44), the amount of $2,500,000 is appropriated for the state fiscal year beginning September 1, 2015, and the amount of $2,500,000 is appropriated for the state fiscal year beginning September 1, 2016, from the general revenue fund to the Department of State Health Services for the purpose of implementing and expanding the United States Centers for Disease Control and Prevention's evidence-based National Diabetes Prevention Program.
