(1)  On page 33, line 2, after the underlined semicolon, add "and".

(2)  On page 33, line 4, strike the underlined semicolon.

(3)  On page 33, strike lines 5 through 8.

(4)  On page 33, line 9, strike "twice in a five-year period".
