Amend CSHB 5 (house committee printing) on page 13, between lines 23 and 24, insert the following appropriately numbered subdivision and renumber subsequent subdivisions:
(____) a sworn affidavit disclosing if the applicant has ever required an employee or contractor to receive a vaccination and if the vaccination was compelled by government action.
