Amend CSSB 14 (house committee report) as follows:

(1)  On page 1, line 22, strike "dispense or".

(2)  On page 2, line 25, strike "provide, prescribe, administer, or dispense", and substitute "prescribe".

(3)  On page 3, strike lines 10 and 11, and substitute "apply to a physician or health care provider, with the consent of the child's parent or legal guardian, prescribing or providing:".

(4)  On page 3, line 27, through page 4, line 1, strike "the provision of a prescription drug to", and substitute "prescribing a prescription drug for".

(5)  On page 4, line 16, strike "providing to", and substitute "prescribing for".

(6)  On page 4, line 23, strike "or facilitates the provision", and substitute ", prescribes, or facilitates the provision or prescription".
