Amend HB 4730 (house committee report) as follows:

(1)  On page 1, line 10, between "by" and "the", insert "the Health and Human Services Commission in conjunction with".

(2)  On page 1, line 20, strike "Department of Family and Protective Services" and substitute "Health and Human Services Commission, in conjunction with the Department of Family and Protective Services,".

(3)  On page 1, line 21, strike "the department's" and substitute "each agency's".

(4)  On page 3, lines 14 and 15, strike added Section 162.00639(a), Family Code, and substitute the following:

(a)  In this section:

(1)  "Commission" means the Health and Human Services Commission.

(2)  "Department" means the Department of Family and Protective Services.

(5)  On page 3, lines 16 and 17, strike "department shall adopt and make available on its publicly accessible Internet website" and substitute "commission, in conjunction with the department, shall adopt".

(6)  On page 3, line 18, between the underlined period and "The", insert "The commission and the department shall make the form available on each agency's publicly accessible Internet website."

(7)  On page 5, lines 19 and 20, strike "Department of Family and Protective Services" and substitute "Health and Human Services Commission, in conjunction with the Department of Family and Protective Services,".
