Amend SB 1257 (house committee printing) by striking page 3, line 26 through page 4, line 2 and substituting the following:
(c) This chapter applies to coverage under a group health benefit plan provided to a resident of this state only to the extent that the procedure or treatment described by this chapter is administered in this state.
